Bannu: The home ministry on Monday dispatched preliminary inquiry report into the Bannu jail attack to the federal interior ministery, describing it an intelligence failure.
According to the report 150 attackers on 50 vehicles entered the area and stormed the central jail for two hours.
The reports said that 378 prisoners who fled the jail include 269 convicted and 89 accused inmates while 5 women prisoners also escaped.
The report noted that 30 policemen including 10 armed personnel were present while 63 were absent from the duty at the time of the attack.
It said mobile phones were also being used by inmates without any check by the jail authorities.
According to the report, the prisoners also include Adnan Rashid, mastermind of an attack on former military ruler Pervez Musharraf, and a Taliban commander Nooruddin.
